The various forms needed for the International Lodges are downloadable from this page.

※ATTENTION: Residents of the Mejirodai International Village are to use the forms
                        provided by the Mejirodai management company and not the ones below.
                        For the details, please check the move-in guidance email sent to you
                        from the Mejirodai management company.

■BEDDINGS PURCHASE ORDERING FORM

!This form is not intended for those moving in to the Mejirodai International Village.
   If you are to reside at Mejirodai, please be sure to order your beddings from the 
   URL provided in the email sent from the Mejirodai management company.

 

For the placement of your orders for the beddings, please send the filled out form to the accommodation office at least 10 days prior to your actual move-in date.
*Purchase of the bed pad(s) is required for all residents.
  Even though you are to prepare your own beddings, be sure to place your order
  for the must-buy bed pad in time.
*The fees for the bedding sets may change slightly from April 1, 2025. The detailed 
  information will be updated around mid-February 2025.

Directly send the forms to the accommodation office and NOT to the ֱapp Housing Office.

Please complete this form and submit it to the Lodge Office within seven days from the date of your arrival.  If you should fail to submit this form, you may have to take the responsibility for damages caused by the previous resident.

■OTHER NECESSARY DOCUMENTS


To prepare for an emergency, the Lodge Office should always be able to contact every resident. If you plan to stay overnight outside the Lodge, please be sure to fill out and submit the “Notice of Travel / Absence from Lodge” to the Lodge Office in advance.

■DOCUMENT NEEDED AT THE TIME OF DEPARTURE

You are free to leave the Lodge before the expiration date of your term of residence as long as you fill out the “Notification of Departure of the University of Tokyo International Lodge” and submit it to the Lodge Office no later than 14 days prior to your actual date of departure.  Otherwise, you will be billed of all fees up till the 14th day from the date of submission (no matter if you are to move out of the Lodge at an earlier date than that).

■ ISSUANCE OF PROOF OF RESIDENCE AT ACCOMMODATION

If you need a certificate of payment/residence for the period of your stay at the dorm, please complete the application form below and submit it to the ֱapp Housing Office. 

  ・Send to: housing-office.adm@gs.mail.u-tokyo.ac.jp
  ・Subject e-mail as: Request for Proof of Residence: Your Room Number・Name in full

Certificates are issued only for clear and valid reasons, such as submission to an employer, taxation office, or relevant institutions in your home region/country.
※Basically, certificates can only be issued twice a year per person.  If you are to have any special reason, please priorly consult the ֱapp Housing Office.
